Endometriosis is one of those conditions where the gap between first symptoms and confirmed diagnosis has historically been unacceptably wide.

What Is Endometriosis?

Endometriosis is a condition where tissue similar to the womb lining grows outside the uterus, most commonly on the ovaries, fallopian tubes, pelvic lining, bladder, and bowel.

Because this tissue responds to the hormonal cycle, it bleeds each month in the same way the womb lining does.
But unlike a normal period, the blood has nowhere to go. The result is inflammation, scarring, and often significant pain.

It is not a rare condition. It affects roughly one in ten women of reproductive age.
When it goes undiagnosed for years, it can affect fertility, cause progressive structural damage and severe pain.

What are the symptoms of endometriosis?

Symptoms that may suggest endometriosis include:

  • Chronic pelvic pain, both during and outside of menstruation
  • Severe period pain (dysmenorrhoea) that worsens over time or does not respond to painkillers
  • Painful sex (dyspareunia)
  • Pain during defecation or urination
  • Blood in stools or urine during menstruation
  • Right shoulder pain during periods, which can indicate diaphragmatic involvement
  • Bloating and nausea that follow the menstrual cycle
  • Digestive issues
  • Unexplained difficulty conceiving

Severe, disruptive menstrual pain is not a normal part of being a woman.

Why Has Endometriosis Been So Difficult to Diagnose?

The core problem is that the only definitive way to confirm endometriosis has been laparoscopy, which is a keyhole surgical procedure under general anaesthetic, enabling a surgeon to literally see inside the pelvis, and take biopsies.

Ultrasound and MRI can’t reliably detect all forms of endometriosis. So far, a women’s symptoms had to reach a threshold that justified surgery before a diagnosis could be confirmed.

In practice, this has meant years of consultations, often with multiple different clinicians, before anyone joined the dots. Many women have felt dismissed by the medical community, being told their pain is ‘normal’. Some are given treatments that do not address the underlying condition.

Laparoscopy still confirms the diagnosis. But the Ziwig Endotest means we stop wasting years getting to that point.

How Does the Ziwig Endotest Work?

Your saliva contains molecular signals that reflect what is happening in your body.
In women with endometriosis, those signals follow a recognisable pattern.
The Ziwig Endotest uses artificial intelligence to analyse 109 of these markers and determine whether that pattern is present.

The published evidence is strong. The test correctly identifies endometriosis in 97 out of 100 women who have it, and correctly rules it out in 93 out of 100 who do not. We always interpret results alongside your symptoms rather than in isolation, but as non-invasive diagnostic tools go, those are impressive figures.

Who is the Ziwig Endotest for?

The test is for women aged 18 to 43 who have symptoms that could point towards endometriosis.
If you have been experiencing unexplained pelvic pain and want clearer answers, this test may be a useful next step.

It is likely appropriate if you have:

    • Persistent pelvic or period pain that’s getting worse or not responding to painkillers
    • Pain during sex
    • Bowel or bladder symptoms that follow your cycle
    • Blood in the stool or urine during your period
    • Symptoms that have been investigated but never properly explained
    • Difficulty getting pregnant where endometriosis has not been ruled out

The test is not appropriate if you are pregnant, outside the age range above, HIV positive, or have a history of cancer.

Does the Ziwig Endotest Replace Laparoscopy?

No. Laparoscopy remains the gold standard for confirming endometriosis, and that has not changed. What this test changes is how long women wait before the process gets moving.

A negative result is equally useful. If endometriosis is unlikely, we can focus our investigations on other causes of pelvic pain.
