Medication Review
A structured medication review is a great way to take a fresh look at your medications, particularly if you have a long-term health problem (such as high blood pressure), multiple medical conditions, or are taking several medications.
We believe in helping you to understand not only the medications you’re currently taking, but why you’re taking them.
As our health changes, some medications may no longer be as suitable as they once were for us, and sometimes side effects can be reduced by switching to alternatives.
If you’ve recently been in hospital, have had a new medical diagnosis, or if it’s been many months since a Doctor closely looked at your medications, it’s an ideal time to have a medication review.
During the review, you’ll also be asked about any over-the-counter medications you’re taking and what you’re using them for. Additionally, if your medication requires any monitoring, such as blood testing, our doctors will discuss that with you as well.

To prepare for the medication review, we recommend that you make a list of all the medications you’re taking, have been prescribed, or you have chosen to take yourself. Feel free to bring your medications, pills, sprays and liquids with you, and make a note of any side effects you think you may be experiencing, and all of the questions that you have.
Together with your Doctor, an individualised medication plan will be developed, and you’ll be given clear instructions about your medications, and how to take them. We want you to be happy with your treatment, and feel confident in using your medications.
